What using this website means: a request is not a booking, how the deposit works, and where our responsibility begins and ends.

The deposit refund, the fallback when Square cannot take the deposit, cancellation and no-show terms, the acceptance-by-use clause, the governing-law clause, the liability wording and the effective date are placeholders to confirm.
These terms cover your use of quawellness.com, the website of Qua Wellness and Med Spa, a physician-led wellness and aesthetics practice at 430 Village Center Drive, Burr Ridge, IL 60527. They are written in plain English because we would rather you read them than skip them. By using the site, requesting an appointment or asking a question through Ask Qua, you agree to them.
They cover the website only. The care you receive in the practice is governed by the consent forms you sign with us and by the clinical relationship between you and Dr. Abboud and her medical team. Your clinical records are protected under HIPAA and described in our Notice of Privacy Practices. How this site handles your information is set out in our Privacy Policy.
A request is not a booking
The concierge page lets you request an appointment. You choose a treatment, a preferred date and time, and how you would like us to reach you, and you agree that we may contact you about the request by phone, text or email. That is a request. It is not a confirmed appointment.
A $50 deposit through Square holds your request. Our team then confirms every appointment personally, using the contact method you chose. Until you hear from us, you do not yet have an appointment.

The deposit
The deposit is $50 and it is paid through Square. Your card details are entered on Square's checkout, never on this website, and Square handles them under its own terms. The details of your request are attached as the note on the Square payment, so our team sees them together with the deposit.
If the deposit page cannot be opened, for example when Square is unavailable, your request still reaches us straight away, and we will contact you to take the deposit by phone and confirm the time. If you leave Square's checkout without paying, your request is not sent to us; you are welcome to start again or to call.
The deposit goes toward the cost of your visit. If we cannot confirm the time you asked for, and we cannot agree on another, the deposit is returned in full.

The deposit is the only price stated on this website. Treatment pricing is given at your consultation, where you leave with a plan and exact pricing, not a package. Ask the front desk about insurance for medical visits.
Not medical advice, on the page or from Ask Qua
Everything on this website is general information. It describes what we offer and how we think about care. It is not medical advice, and it is not a substitute for a consultation with a physician who has seen you, taken your history and read your labs. What helps one person may not suit another, which is what an evaluation is for.
Reading this site, requesting an appointment or using Ask Qua does not make you a patient of the practice. That relationship begins in the practice, at your first visit.
Ask Qua is the assistant on this site. It answers questions about the practice. When you type a question, the question and the conversation so far are sent through the Vercel AI Gateway to an AI model, which produces the answer. Its answers are general information, not medical advice. Like any automated assistant, it can be wrong, incomplete or out of date. Please check anything that matters with us, and treat what we tell you in person as the final word.
Please do not put health details in the scheduling note or in Ask Qua. Those fields are for scheduling and general questions, and they are not designed to hold medical information. The text of Ask Qua questions may be kept, with email addresses and phone numbers blanked and no name or contact detail attached, so we can improve the answers. If you use the Leave a number button in Ask Qua, your name, contact detail and question are emailed to us so that a person can follow up. If there is something medical we should know before your visit, call us at (630) 561-0590.
If you think you are having a medical emergency, call 911 or go to the nearest emergency department. This website is not for emergencies.
Content, links and acceptable use
Every image on this site is of Qua. Photos of our rooms are real, with retouched lighting. Some treatment and service images are illustrations set in our real rooms. The opening portrait of Dr. Abboud is a composite made from her real portrait and our real reception. Videos, portraits and patient photos are real and shared with permission. Patient stories describe those patients' experiences, and they are not a promise of what will happen for you. Results vary.
The words, photographs, logo and design of this site belong to Qua Wellness and Med Spa or are used with permission. You are welcome to read, print and share pages for your own personal use. Please do not copy, sell or republish them, or present them as your own. Treatment and device names on this site, such as Botox, Morpheus8 and CoolSculpting, belong to the companies that make them, and we use them only to describe what we offer.
Some links take you off this site: Square for the deposit, the 360 tour on tourmkr.com, our Instagram and Facebook pages, and Google Maps for directions. Each of those services has its own terms and privacy policy, and we do not control them. Once you leave this site, their terms apply, not ours.
We ask that you use the site as it is meant to be used. Please do not request appointments you do not intend to keep, enter another person's details without their permission, or try to interfere with the site, its forms or Ask Qua. We may decline a request that appears to be false or misused.
Our responsibility, your information and the law
We take care to keep this site accurate, but hours, services, availability and the details of treatments can change, and a page may sometimes be out of date or unavailable. We do not promise that the site will always be available, complete or free of errors.
The site is provided as it is. To the extent the law allows, we are not responsible for loss or harm that comes from relying on general information on the site, from an answer given by Ask Qua, from an interruption or fault in the site, or from a service we do not run, such as Square, Vercel, Resend, the services that alert our team to a request, or the social networks we link to.
Nothing in these terms limits any responsibility that cannot be limited under Illinois law. Nothing here changes our duties to you as your medical provider once you are a patient. Those duties are set by law and by the consents you give in the practice, not by this page.
For the detail on your information, read our Privacy Policy. In short: the site sets no cookies of its own, and Google Analytics 4 records page views and clicks only, never what you type into a form. Apart from the text of Ask Qua questions, described above, nothing you enter is stored on the website's own servers. The website is not connected to the electronic health record we use in the practice. If you sign up for notes from Dr. Abboud in the footer, every email carries an unsubscribe link.
We may update these terms from time to time. The version on the site when you use it is the one that applies, and the effective date below tells you when it last changed. These terms are governed by the laws of the State of Illinois. If any part of them cannot be enforced, the rest still applies.

Choose a treatment and a preferred date and time on the concierge page. No referral is needed. The scheduling note is for scheduling only, so please leave out medical details.
- Pay the deposit
Square's checkout takes the $50, and your card never touches this website. Once the payment goes through, your request reaches our team together with the deposit.
- We confirm with you
A member of our team calls, texts or emails you, whichever you chose, to confirm. Until then, the time is a request, not an appointment.

Not yet. The deposit holds your request. You have an appointment once a member of our team has confirmed it with you.
It is credited against your visit. If we cannot confirm a time with you, we return it in full. Our cancellation, rescheduling and no-show terms will be stated here once the practice has confirmed them.
Ask Qua gives general information about the practice and can be wrong. It is not medical advice. Check anything that matters with us before you rely on it.
The website only. Your care in the practice is a clinical matter between you and Dr. Abboud's medical team, set out in the consent forms you sign there.
